CASE MANAGER

PURPOSE AND POSITION SUMMARY

The Case Manager at A House of Blessing works directly with families with disabilities exiting homelessness, empowering them with skills for greater independence in the areas of goal setting, parenting, finance, employment, education, and housing stability. The case manager is responsible for proper assessment, providing individual and group interventions, and evaluating clients’ progress toward permanent housing. The Case Manager represents the professionalism and dedication that is associated with Beatitude House.

QUALIFICATIONS

  • Bachelor’s degree in social work or equivalent field
  • Excellent communication skills
  • Ability to work independently
  • Willingness to be versatile
  • Capable of making decisions
  • Proficient in crisis management
  • Experience in providing individual and group services to clients
  • Experience in utilizing community resources
  • Dedicated to ongoing personal and professional growth
  • Valid driver’s license
  • LSW or LISW and two years of client-related services preferred

RESPONSIBILITIES

To ensure that the mission and purpose of Beatitude House is accomplished, the Case Manager will:

  • Carry out all policies and procedures of Beatitude House
  • Participate in staff meetings, as scheduled
  • Supervise interns as needed
  • Establish positive partnerships with area resource providers
  • Abide by code of professional ethics
  • Participate in public relations activities as requested
  • Maintain professional growth through goals set annually
  • Advise Scholars staff on referrals and social service resources for Scholars Program students
  • Provide services as requested by the Support Service Director

To ensure program compliance with all grant and organization regulations for client eligibility, the Case Manager will:

  • Coordinate referrals from CEARS
  • Assess clients based on eligibility guidelines
  • Facilitate program admission, completing all required intake documentation and entry into databases with clients
  • Stay updated on all government and grant regulations
  • Maintain proper record keeping and database entry to ensure grant compliance

To carry out the housing stability plans with clients, the Case Manager will:

  • Schedule a session every 90 days with appropriate staff to review and plan goals
  • Document goals, interventions, and accomplishments in records and databases
  • Develop appropriate weekly family interventions to remove barriers to permanent housing
  • Coordinate services with other providers and monitor involvement
  • Facilitate individual and group meetings with clients

To facilitate client’s transition to permanent housing, the Case Manager will:

  • Assist client with appropriate housing and community resources
  • Complete exit documentation and entry into databases
  • Continue supportive contact for up to six months after client’s exit

To ensure continuity of care for our clients, the Case Manager will

  • Collaborate with other site staff on event and program planning for clients
  • Provide transportation for participants as needed
  • Prepare Incident Reports on situations that may have legal and/or organizational ramifications
  • Carry and respond to an on-call agency phone on a rotating basis
